Referrals

Currently we are only taking school-based referrals for youth from Grand Mesa Middle School and Bookcliff Middle School. We ask that youth are only referred by school counselors/mental health professionals. Our mentoring program is best fit for youth who are dealing with adverse experiences; please keep referrals limited to youth in that category.

Referral Form

Before we can accept any referrals, we are required to have a Release of Information form filled out by the youth's parent/guardian.
